New Patient Registration - 

As of Monday 19th March 2023, we will only be able to accept registration forms from new patients who have moved into the Paisley area and are not already registered with a local GP practice. Any patients who are currently registered with a GP practice within our postcode area (see below) will not be able to register at the Greenlaw Practice at the moment.  

From 19th March 2023 the practice will only accept patients who reside in the following Postal Codes: -

PA1        Paisley East

PA2        Paisley South

PA3        Paisley West/North (PA31,PA32 and PA34) Not PA33

We register Patients daily. Ideally New patients will be required to provide a form of identification, although non production will not deny your registration with the practice. 

During the process a staff member may then contact you to obtain details of your medical history and current medication whilst we are waiting for your  records from your previous doctor.

If you wish to register click on the link below to open the form. When you have completed all of the details, please email your form to ggc.gp87541@nhs.scot  When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

This is because it can take a considerable time for us to receive your medical records. Please complete the questionnaire below and click on the 'Send' button. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.
